Accessing 1095-A forms

Retrieve 1095-A forms for clients using the HealthSherpa dashboard.

A Form 1095-A is the Marketplace tax form that shows a consumer’s coverage months and premium amounts, including any advance premium tax credit (APTC) paid, and it’s used to reconcile the APTC on the consumer’s federal tax return.

Agents can retrieve 1095-A forms for clients directly through the HealthSherpa dashboard.

In this article we’ll cover:

1095-A forms are not accessible from HealthSherpa Enrollee Assistance Full-Service account types.


Where 1095-A forms appear in HealthSherpa

Select the consumer’s name on the Clients page to open their consumer record.

The Details tab is the default view when an agent opens a consumer record from the Clients page.

Agents can only service an application and view the most up-to-date application information when they have an active Enhanced Direct Enrollment (EDE) sync to the application.

From the client's Details page, the 1095-A can be downloaded from the Marketplace notices section or the Enrollment history section.


Accessing a 1095-A for consumers not on the Clients page

To assist a consumer who is not already on the Clients page, with consent, agents can search & claim the consumer’s existing application.

After the application is claimed, the policy status may show as Terminated. This is expected for enrollments from a prior plan year.


Troubleshooting when a 1095-A is missing

If the 1095-A is missing or unavailable, use the tips below to determine what to do next.

Certification requirements for prior plan years

Agents can typically access 1095-A forms only for plan years when they were Marketplace certified. If Marketplace certification was not active for that plan year, the 1095-A may not be accessible through HealthSherpa.

This year-of-certification access also commonly applies when a state transitions from the Federally Facilitated Marketplace (FFM) to a State-based Marketplace (SBM).

Example:

  • An agent is FFM certified for plan year 2025, but was not FFM certified for plan year 2024.

  • The agent may not be able to retrieve a 1095-A form for plan year 2024 through HealthSherpa.

  • The client may need to contact the Marketplace responsible for plan year 2024 to request the form.

When the Marketplace releases 1095-A forms

It’s common for the Marketplace to release 1095-A forms in batches at the start of the year. Forms are often issued over several weeks.

HealthSherpa links 1095-A forms to client applications as soon as the Marketplace makes them available. This can result in some clients having a 1095-A available while others do not.

If a 1095-A is not available by the first week of March, the client should contact the Marketplace responsible for that plan year to request the form.

Contact the correct Marketplace

1095-A forms are tied to the prior year’s coverage. Questions should be directed to the specific Marketplace that oversaw coverage for that plan year.

Example:

  • A consumer has questions about 1095-A forms for coverage in Georgia.

  • Georgia used the Federally Facilitated Marketplace (FFM) for plan year 2024.

  • Georgia transitioned to a State-based Marketplace (SBM) beginning in plan year 2025 (Georgia Access).

  • Questions about 1095-A forms for plan year 2024 should be directed to the FFM.

  • Questions about 1095-A forms for plan year 2025 should be directed to Georgia Access.
